A friend or partner who notices you're grinding and physically removes you from the situation does more for you than any vacation you'd plan yourself, because you wouldn't plan one that actually rests you. You need at least one relationship where the intensity you usually reserve for work gets to go somewhere with no output attached, no metric, no eventual payoff, just contact.
Quiet, repetitive physical ritual helps more than you'd expect. Something with your hands, away from screens and scoreboards, that gives the Saturn part of you a container to work inside without asking what it's for.
Advice for the Road
Pick one relationship or one hour a week where the goal is not to accomplish anything, and defend it the way you'd defend a deadline. You already know how to protect things that matter. Start protecting rest with the same discipline you protect your ambition, or the ambition will eventually be all that's left standing.
